You'll be joining a fast-paced, multidisciplinary team where your expertise will make a genuine difference in people’s lives. This specialist role focuses on supporting vulnerable adults with alcohol and/or substance misuse issues during and after discharge—helping them access the right community services, prevent readmission, and achieve long-term recovery.
