Oxleas NHS Foundation Trust seeks an experienced Primary Care Clinical Lead to drive the delivery and development of high-quality primary care services in our Bristol prison healthcare setting. This role combines clinical leadership, operational management and service improvement to directly improve patient care and outcomes.

You will lead a multidisciplinary team, oversee staffing and rotas, and ensure safe, efficient, and responsive services.

Aufgaben

• Provide clinical and operational leadership for the primary care service within a prison health setting.
• Manage, support, supervise and appraise staff, promoting professional development and high standards of practice; coordinate staffing and rotas to ensure safe service delivery.
• Oversee referral, triage, care coordination and clinical review processes; maintain a small clinical caseload and provide evidence-based care for patients with acute, complex and long-term health needs.
• Lead quality improvement, clinical audit and service evaluation activity; monitor performance, patient safety and standards of care; take action to drive improvements.
• Collaborate with HMPPS, healthcare partners, community providers and other agencies to support integrated care and continuity of treatment.
