We’re looking for an exceptional Primary Care Transformation Manager to lead and deliver strategic improvements across our GP practice. This is more than a management role it’s a unique opportunity to shape the future of healthcare delivery in your community.
About the Role
As our Primary Care Transformation Manager, you’ll be at the heart of change leading key initiatives that modernise general practice, improve patient care, and align with NHS and OneMedicare priorities. You’ll work closely with practice teams, Primary Care Networks (PCNs), Integrated Care Boards (ICBs), and other stakeholders to ensure seamless, high-quality service delivery that meets the needs of the local population.
Your leadership will help embed efficient systems, support income generation, ensure compliance, and most importantly, make a meaningful impact on the lives of our patients.

Rosedean House Surgery is a well-established GP practice based in the heart of Liskeard, Cornwall. The building has served the local community since 1874 and is now home to a dedicated team, providing care to approximately 10,000 registered patients.
