Job description

As a Primary Care Nurse, your responsibilities will vary. You will work within a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions.

Your key responsibilities include:

  • Undertaking medication rounds, dispensing controlled medication, attending emergency response situations, assessing patients, and undertaking reception screening
  • Delivering nurse-led clinics to your patient group whilst being supported by a multi-disciplinary team
  • Providing outstanding evidence-based care to our patients, seeking out, listening to, and acting on their feedback, so that care is personalized and informed by what matters to them
  • Delivering non-judgmental care that makes a real difference
  • Inspiring excellence by forging strong links with our partners and stakeholders
  • Promoting best practice, clinical supervision, evidence-based care, and continuous quality improvement, embedding a culture of shared learning
  • Sharing knowledge, skills, and expertise to ensure safe care, building strong multi-disciplinary teams

This role is based on 37.5 hours per week, with a combination of nights and weekends, including some bank holidays. You will receive an annual salary up to £41,659 per annum FTE, depending on experience.
