Job description

We have an exciting opportunity for a Primary Care Nurse to join our Primary Care team at HMP Swinfen Hall, a category C men's Prison and Young Offenders Institution located in Staffordshire. Opportunity to develop in this role across different areas.

Hours

37.5 hours working 4 days a week, with 1x 7.30am-7.30pm shift and 3x 7.30am-5.30pm shifts, plus every other weekend. 6-week rolling rota.

This is a full-time position with an annual salary up to £36,000 per annum FTE, depending on experience.

Main duties of the job

As a Primary Care Nurse, your responsibilities will include:

  • Working within a multidisciplinary team to deliver nurse-led services and prevent hospital admissions
  • Undertaking medication rounds, dispensing controlled medication, attending emergency situations, assessing patients, and conducting reception screening
  • Delivering nurse-led clinics supported by a multidisciplinary team
  • Providing evidence-based, personalized care based on patient feedback
  • Delivering non-judgmental care that makes a difference
  • Building strong links with partners and stakeholders
  • Promoting best practices, clinical supervision, and continuous quality improvement
  • Sharing knowledge and ensuring safe care within a team environment
