Job summary

Practice Plus Group Holdings Limited is seeking a Primary Care Nurse to join the Primary Care team at HMP Swinfen Hall on a part time basis. You will work in a multidisciplinary team to deliver nurse-led services, conduct medication rounds, and run nurse-led clinics while providing high-quality care in a secure environment.

With 28.5 hours over a 3-day week, the role offers a supportive induction, ongoing development, and a competitive salary.

Responsibilities

• Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
• Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team
• Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them
• Deliver non-judgemental care that makes a real difference
• Inspire excellence by forging strong links with our partners and stakeholders
• Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning

We have an exciting opportunity for a Primary Care Nurse to join our Primary Care team at HMP Swinfen Hall on a part time basis. HMP Swinfen Hall is a young offenders prison home for up to 600 males. This is a great opportunity for an individual to be part of a close knit small team with lots of opportunity to work autonomously over multiple areas of the site picking up great experience along the way.

The site currently offers 28.5 hours over a 3 day week and a 6 week rolling rota.

• Monday to Thursday - 07.30 - 19.30
• Friday to Sunday -07.30 - 17.30, This position is part time, youll receive an annual salary up to £31,660 per annum.
