Practice Plus Group’s Primary Care team invites a dedicated Primary Care Nurse to join the HMP Oakwood site in Wolverhampton. You will deliver nurse-led services within a multi-disciplinary team, supporting healthcare delivery in a secure environment.

You’ll undertake medication rounds, run nurse-led clinics and provide compassionate, evidence-based care while developing professional skills through bespoke training and regional progression opportunities.

Qualifications

• Registered nurse – Adult NMC.
• Evidence of Continued Professional Development (CPD).
• Understanding of multi-disciplinary and multi-agency working.
• Non-judgemental and compassionate approach.
• Excellent communication skills.

Responsabilités

• Work within a multi-disciplinary team to deliver a nurse-led service.
• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients and undertake reception screening.
• Use your skills to deliver nurse-led clinics to your patient group with support from a multi-disciplinary team.
• Provide outstanding evidence-based care to patients, listening to and acting on their feedback so care is personalised.
• Deliver non-judgemental care that makes a real difference and inspire excellence by partnering with stakeholders.
• Promote best practice, clinical supervision, and continuous quality improvement, embedding a culture of shared learning.

We have an exciting opportunity for a Primary Care Nurse to join our Primary Care team at HMP Oakwood, a category C Prison located in Wolverhampton.

HMP Oakwood is one of the largest prison in the UK, housing over 1,500 men.

The site offers:

• Free Parking
• Gym Access

Hours: 37.5

• Monday- Sunday - 10.5 hour shifts
• A fixed 8 week shift pattern
• Every other weekend. 10 -12hr shifts
• No nights

Salary:

Salary range between £34,428 - £41,659 per annum, depending on experience.

What you'll be doing

As a Primary Care Nurse your responsibilities will vary. You will

• Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
• Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
• Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
• Deliver non-judgemental care that makes a real difference
• Inspire excellence by forging strong links with our partners and stakeholders
• Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
• Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.
