Job summary

Practice Plus Group is inviting applications for a Primary Care Nurse to join our Primary Care team at HMP New Hall, a female prison in Flockton, West Yorkshire. You will be part of a fully integrated Healthcare team with a supportive environment.

The role offers 37.5 hours per week with a mix of day and night shifts, including one weekend per month. Salary up to £41,659 per annum FTE depending on experience. Training and development opportunities are provided.

Qualifications

• Registered nurse – Adult is required.
• Current professional qualification with NMC registration.
• Evidence of Continued Professional Development (CPD) is expected.
• Understanding of multi-disciplinary and multi-agency working is important.
• Non-judgemental and compassionate approach is essential.
• Excellent communication skills are required.

Responsibilities

• Deliver a nurse-led service within a multi-disciplinary team to prevent hospital admissions.
• Undertake medication rounds and dispense controlled medications.
• Attend emergency responses and perform patient reception screening.
• Deliver nurse-led clinics and provide evidence-based care tailored to patients.
• Provide non-judgemental care and seek patient feedback to inform care.
• Forge strong links with partners and stakeholders to promote best practice.
• Contribute to continuous quality improvement and shared learning.

About The Role

Hours

37.5 hours a week, 3 shifts working a mixture of days 6.45am-7.30pm or nights, 7.30pm-6.45am. One weekend a month and ad-hoc bank holidays.

Salary

This position is full time, you’ll receive an annual salary up to £41,659 per annum FTE - depending on experience.

What you'll be doing

• Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
• Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
• Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
• Deliver non-judgemental care that makes a real difference
• Inspire excellence by forging strong links with our partners and stakeholders
• Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
• Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.
