Primary Care Network Pharmacy Technician - Woodsley & Holt Park PCN

Leeds Community Healthcare NHS Trust is offering an opportunity for a Band 5 Pharmacy Technician to join the team at Woodsley and Holt Park Primary Care Network (PCN). The PCN serves over 115,000 patients across ten GP practices in North Leeds, focusing on high-quality, personalized healthcare.

As a member of our multidisciplinary team, you will support the safe and effective delivery of medicines-related care. You will collaborate with pharmacists, GPs, nurses, social prescribers, and other health professionals to optimize patient outcomes, support long-term condition management, and reduce medication-related harm.

Main duties of the job

You will provide medicines-related support across ten practices, supporting clinical medication reviews, managing repeat prescriptions, patient education, and adhering to prescribing standards. Your tasks include reviewing medicines post-hospital discharge, handling medication queries, supporting medication reviews, and implementing prescribing guidance. You will contribute to audits, patient engagement, and ensure a person-centered, evidence-based approach, balancing patient contact and administrative work.

The role requires self-motivation, organization, and confidence working independently and in a team to ensure patients receive the right medication at the right time.
