Job description

Primary Care Network Nurse (Diabetes Team)

We are currently recruiting for two positions, with a total of 60 hours per week available. We are happy to consider one full-time equivalent (FTE) position and one part-time position, or alternatively, as two part-time roles, depending on candidate availability and service requirements.

Working collaboratively with practice nurses, health and wellbeing coaches, and our PCN Pharmacist, the Network Nurses will play a key role in supporting and enhancing diabetes management across the network. This position is designed to complement existing services and expand our offer to patients through both individual and group-based interventions.

Main duties of the job
  • Facilitate and lead group sessions, including initiation of GLP-1 therapies (e.g., Mounjaro)
  • Provide ongoing education on diabetes self-management, including diet, exercise, and medication adherence, supported by our established wellbeing coach team and PCN Pharmacist
  • Deliver pre-diabetes group sessions
  • Support in-practice diabetes clinics, working alongside and in support of Practice Nurses

Job responsibilities

Job Title: Network Nurse (Diabetes Team)

Reports To: PCN Manager / Lead Nurse

Hours: Up to 60 hours per week across two positions (flexible full-time and/or part-time arrangements considered)

Salary: Dependent on experience; aligned with the ARRS reimbursable rate (Band 5 or 6 equivalent)

Job Purpose: The Network Nurse will be responsible for supporting and enhancing diabetes care across SAPA 5 Primary Care Network (PCN). Working collaboratively with multidisciplinary teams, the post holder will deliver patient-centred care, provide education, and help implement evidence-based interventions to improve health outcomes for individuals living with or at risk of diabetes.

Key Responsibilities
  • Facilitate and lead group education sessions, including the initiation of GLP-1 therapies (e.g., Mounjaro)
  • Provide education on diabetes self-management, including diet, exercise, and medication adherence
  • Deliver group sessions for patients at risk of diabetes (pre-diabetes)
  • Support in-practice diabetes clinics, working alongside practice nurses and other primary care staff
  • Collaborate with PCN wellbeing coaches and pharmacists to ensure holistic care
  • Maintain accurate and up-to-date clinical records
  • Participate in service development and quality improvement initiatives
  • Attend PCN team meetings and contribute to shared learning
Other Requirements
  • Must have a full UK driving licence and access to own transport
  • Ability to travel between practices within the network
Person Specification
Qualifications
  • A Registered Nurse with experience in diabetes management within a general practice setting, or a willingness to undertake training to develop these skills
  • Confident in initiating and managing GLP-1 therapies or willing to undertake relevant training
  • Strong communication skills, with experience leading patient education sessions
  • Able to work flexibly across practices and within a multidisciplinary team
  • Passionate about improving long-term outcomes for patients living with or at risk of diabetes
  • Must have own transport and a full driver’s license
  • GP practice experience is desirable but not essential
