A leading healthcare organization is seeking a qualified Clinical Pharmacist to join their multi-disciplinary team. If so this is anexciting opportunity for you to join our existing pharmacy team, working with general practices across our PCN footprint,with support from our existing clinical teams to deliver excellent patient care. You will be partof our growing multi-disciplinary additional roles team, who all work togetherto support our GP practices and their patients.The rolescurrently include Care Coordinators, Clinical Pharmacists, PharmacyTechnicians, Paramedics, Health and Wellbeing Coaches, Social Prescribing LinkWorker, Outreach Nurses and Mental Health Practitioner.
The post holder will work within their clinical competencies as part of amulti-disciplinary team to provide expertise in clinical medicines management,provide face-to-face structured medication reviews, manage long termconditions, management of medicines on transfer of care and systems for saferprescribing, manage repeat prescription authorisations and reauthorisation,acute prescription request, while addressing both the public health and socialcare needs of patients in the GP practice(s) that make up the PCN.
The base for this role will be at Henry Moore Clinic, but working with all six practices within the PCN.
