Job description

This role will be part of the Multi-Disciplinary Team (MDT) at HaxbyGroup. The post holder will contribute to tackling inequalities in health andsocial care. This role will suit an enthusiasticindividual who is able to work collaboratively across a team and keen todeliver within the evolving role of the network.

The Care Co-ordinatorwill be part of a team of care coordinators supporting different cohorts ofvulnerable patients including, but not limited to, residents in care homes,people with learning disabilities, people with severe mental health illness,newly diagnosed cancer patients, carers, people from ethnic communities andpeople with long term conditions.

The post holder willwork alongside the nursing and medical teams in proactively supporting patientsto engage with annual health checks, developing support plans and signpostingand coordinating support from other agencies to improve inequalities in health.

Thepost holder will work across the 4 GP surgeries (Haxby Group PracticeScarborough, Eastfield Medical Centre, Castle Health Centre, and Brook SquareSurgery) who form Scarborough Core PCN.

Main duties of the job
  • Working closely with thePrimary Care Network Transformation Manager.
  • To support the socialprescribing team obtaining information from patients and signposting services.
  • Working on specific projectsdirected by the PCN to address health inequalities.
  • To support existing projects toprovide cover as and when required.
  • To provide administrativesupport for various projects as and when directed.
  • To keep accurate records usingagreed systems and contribute to project evaluation.
  • To work according toconfidentiality and data sharing policies of SCORE PCN.
  • Achieve agreed performancetargets and participate in regular supervision and personal reviews.
  • Engage in training, identifypersonal and professional development needs and continuously seek to refreshknowledge in relation to the job.
  • Cooperate in the formulationand development of systems for monitoring and evaluating the work of theorganisation.
  • Provide information for andcontribute to the administration of communications media as appropriate.
  • To work across any of the 4Practice's as and when required.
  • Work within the ClinicalGovernance framework of the practices in the PCN, incorporating risk managementand all other quality initiatives.
  • Where required and appropriaterefer people back to other health professionals within the primary carenetwork.
  • Support staff within theprimary care network with information about local voluntary and communityresources and how patients can access them.
