A leading primary care organization in Scarborough seeks a Care Coordinator to facilitate support for vulnerable patients. This role involves teamwork within a multi-disciplinary setting to tackle health inequalities, engage patients in health checks, and coordinate services. Ideal candidates will have an NVQ Level 2/3 in Health and Social Care, alongside strong communication and teamwork skills.
This role will be part of the Multi-Disciplinary Team (MDT) at HaxbyGroup. The post holder will contribute to tackling inequalities in health andsocial care. This role will suit an enthusiasticindividual who is able to work collaboratively across a team and keen todeliver within the evolving role of the network.
The Care Co-ordinatorwill be part of a team of care coordinators supporting different cohorts ofvulnerable patients including, but not limited to, residents in care homes,people with learning disabilities, people with severe mental health illness,newly diagnosed cancer patients, carers, people from ethnic communities andpeople with long term conditions.
The post holder willwork alongside the nursing and medical teams in proactively supporting patientsto engage with annual health checks, developing support plans and signpostingand coordinating support from other agencies to improve inequalities in health.
Thepost holder will work across the 4 GP surgeries (Haxby Group PracticeScarborough, Eastfield Medical Centre, Castle Health Centre, and Brook SquareSurgery) who form Scarborough Core PCN.
