An exciting opportunity has arisen to join us as the Team Leader for the Primary Care Mental Health Team (PCMHT) Neighbourhood 2. We are seeking an experienced mental health practitioner, such as a nurse, social worker, or occupational therapist.

The team comprises a diverse multidisciplinary team (MDT). We offer teaching sessions, space for reflective practice, and opportunities to be a champion in your chosen areas.

You will lead the Primary Care Mental Health Team to deliver timely assessments, care planning, and evidence-based health and psychosocial interventions to adults in Lewisham with mental health needs. The successful candidate will oversee interfaces with partner agencies, including Primary Care, Secondary Care, and MIND, to ensure effective service delivery and discharge planning.

The PCMHT now works directly with Primary Care, sharing the same electronic record system, and the successful candidate will receive training. The team operates using our newly developed Care Process Model to ensure standardized care for patients and carers.

Key Responsibilities

  1. Lead and coordinate the work of an integrated neighbourhood Primary Care Mental Health Team, ensuring the provision of safe, effective, high-quality community services in line with statutory requirements and local policies.
  2. Supervise a multidisciplinary, multi-agency team of health, social care, and voluntary sector professionals based in the community.
  3. Ensure efficient triage, assessment, and referral allocation processes.
  4. Maintain oversight of the team's work to ensure continuous flow of service-users through the system.
  5. Enhance liaison with GP practices to maximize collaboration and new links.
  6. Oversee safe working practices and promote effective agile working directives for the team.

The Primary Care Mental Health Teams are integrated services supporting primary care for individuals with complex mental health issues. The team includes psychiatrists, nurses, social workers, employment specialists, occupational therapists, and psychologists, all working in a recovery-focused, strengths-based approach tailored to individual needs rather than diagnosis.
