About the Job

The Primary Care Clinical Pharmacist provides clinical pharmacy services within the primary care network, working with patients and healthcare teams to develop therapeutic plans, monitor drug therapy effectiveness, and resolve drug-related problems.

This is a Relief Full Time (1.00) position, working Monday to Friday, 08:00 to 16:00, providing coverage until the incumbent's return.
