Job summary

A leading healthcare provider is seeking a Prescribing Pharmacist to join a GP surgery in High Wycombe. This 12-month fixed term role focuses on enhancing patient care in chronic conditions and involves a range of clinical responsibilities. The successful candidate will need a strong background in Primary Care and relevant clinics such as Asthma and Diabetes.

Qualifications

• Must be a practicing Pharmacist with experience in Clinical, PCN, or Practice roles.
• Prior experience in managing medications for long-term conditions is essential.

Responsibilities

• Contribute to improving health outcomes in long-term conditions.
• Provide specialist pharmacy knowledge and advice.
• Review patient’s medicine requests and complete clinical documentation.

Job description

I am currently recruiting for a Prescribing Pharmacist to work at a GP surgery based in High Wycombe on a 12-month fixed term contract.

You must have Primary Care experience and ideally have some background in Asthma, Hypertension or Diabetes clinics.

Duties will include:

• Contributing to the improvement of health outcomes in long-term conditions
• Providing specialist knowledge and advice on pharmaceutical matters
• Reviewing patients’ acute medicines requests
• Completing discharge and clinic letters
• Drawing up structured medication reviews as PCN Pharmacist
• Advising and supporting patients with self-limiting conditions
• A practicing Pharmacist with prior experience working as a Clinical Pharmacist, PCN Pharmacist or Practice Pharmacist focussed on delivering excellent clinical care.
• Supporting practice achievement of key Impact and Investment Fund – PCN Pharmacist and QOF targets as required GP Pharmacist or Practice Pharmacist

This is a 12-month fixed term contract, working 34 hours per week, Monday to Friday.
