Qualifications

  • Prior experience as a PCN Pharmacist or Clinical Pharmacist.

Job description

I am currently recruiting for a Prescribing Pharmacist to work at a GP surgery based in High Wycombe on a 12-month fixed term contract.

You must have Primary Care experience and ideally have some background in Asthma, Hypertension or Diabetes clinics.

Duties will include:

Contributing to the improvement of health outcomes in long-term conditions

Providing specialist knowledge and advice on pharmaceutical matters

Reviewing patients' acute medicines requests

Completing discharge and clinic letters

Drawing up structured medication reviews as PCN Pharmacist

Advising and supporting patients with self-limiting conditions

A practicing Pharmacist with prior experience working as a Clinical Pharmacist, PCN Pharmacist or Practice Pharmacist focussed on delivering excellent clinical care.

Supporting practice achievement of key Impact and Investment Fund - PCN Pharmacist and QOF targets as required GP Pharmacist or Practice Pharmacist

This is a 12-month fixed term contract, working 34 hours per week, Monday to Friday.
