Job Summary

We are seeking a dedicated and skilled Prescribing Paramedic to join our Wigan North PCN team. The successful candidate will work collaboratively with multidisciplinary teams to ensure the best outcomes for patients. The role involves being part of a care home team providing support to care homes allocated to the 4 practices within the PCN. The position will be employed under the Additional Roles Reimbursement Scheme (ARRS) meeting the requirements outlined in the Network Contract Agreement DES. Please note, this advert may be closed early depending on the number of applications received.

Main duties of the job
  • Responsible for the management of the care home team and oversee the delivery of the service.
  • Provide a regular ward round to patients at care homes within the PCN.
  • Ensure care homes follow correct procedures in raising daily queries to the appropriate teams to reduce the workload on the practices.
  • Complete and review care plans for all patients within the care homes.
  • Responsible for all medication-related queries and compliance for patients within the care homes.
  • Work alongside the pharmacy technician to review and complete medication reviews for patients within the care homes.
  • Attend meetings with the PCN to provide updates and discuss service.
  • Ensure continued compliance with indemnity cover.
  • Practice in compliance with respective code of professional conduct and within scope of practice, being responsible and accountable for decisions, actions and omissions.
  • Demonstrate a critical understanding of their broadened level of responsibility and autonomy and limits of own competence, including when working with complexity, risk, uncertainty and incomplete information.
  • Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make a positive change.
  • Use expertise and decision-making skills to inform clinical reasoning.
  • Be a professional role model for the PCN.

We are a PCN of four practices in the Wigan North area: Aspull Surgery, Beech Hill Medical Practice, Shevington Surgery and Standish Medical Practice. We work closely together to deliver high-quality patient care.

Job responsibilities

In addition to the primary responsibilities, the Advanced Clinical Practitioner may be requested to:

  • Undertake overall management of a clinical team, providing guidance when necessary, acting as a mentor to students and newly qualified staff
  • Maintain an effective clinical staff rota ensuring all clinics are staffed appropriately
  • Participate in local initiatives to enhance service delivery and patient care
  • Support and participate in shared learning within the PCN
  • Continually review clinical practices, responding to national policies and initiatives where appropriate
  • Participate in the review of significant and near-miss events applying a structured approach (root cause analysis, RCA)
  • Support the development of clinical services within the PCN, liaising with external agencies as required

Quality and Continuous Improvement (CI)

All personnel are required to think about how they achieve results and look for opportunities to improve quality and patient care. Staff should interpret national strategies into local implementation strategies. All staff are to contribute to investigations and root cause analyses and participate in serious incident investigations and multidisciplinary case reviews. The PCN promotes a culture of continuous improvement.

Induction and Learning and Development

We provide a full induction programme. All staff are required to partake in mandatory training as directed. The post holder will mentor team members and disseminate learning. The role supports ongoing professional development and sharing of learning (e.g., courses and conferences).

Collaborative Working and Communication

All staff should recognise collaborative working, manage workloads effectively, and communicate to share information appropriately. Delegation should follow safe practice and competence assessment. Plans and outcomes should be agreed.

Managing Information and Security

Staff should use technology to plan, implement, and monitor care. Data should be recorded with SNOMED/read codes for audit and monitoring. The post holder must adhere to PCN/practice directives and maintain security and confidentiality.

All staff are required to dress appropriately for their role.
