Fountains Medical Practice is seeking a full time Prescribing Paramedic working 37.5 hrs per week over a 5 day working week, providing cover up to 6.30pm. The role will form part of the Fountains Integrated Care Home Service (FICHS) working with care home population.
The successful candidates will be enthusiastic & motivated individual to join our practice team, who will work autonomously within their scope of practice, managing minor illnesses and long-term conditions, including exacerbations of chronic illnesses, i.e. COPD, heart failure. Also contributing to the smooth daily running of the surgery, playing a crucial part in providing on the day management of our patients, for both acute & semi acute illnesses.
The role will be based at the practice in Chester but will include visits to the care homes within the Chester area.
Previous experience working in general practice/primary care is essential.
Undertake full clinical assessment and prescribe autonomously
Able to manage acute and chronic conditions, conduct assessments, and make referrals as appropriate.
Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action.
Makes critical clinical judgements to satisfy the expectations and demands of the job e.g., first contact with patients presenting acutely and manages care in the presence and absence of protocols.
Makes decisions where appropriate without recourse to others e.g. direct referral of patients to hospital consultants without prior reference to other medical colleagues.
Fountains Medical Practice is a well-established NHS GP surgery inChester, with approximately 5000 registered patients including our care home population. We are a friendly, supportive, practice that has as a committed multi-professional team. We currently use the EMIS Web clinical system. Our Team aims to provide healthcare with courtesy, care and efficiency, to all patients.
Conduct assessments, diagnoses, and treatments at first contact (home visits and in-practice). Triage and prioritise patients, including in emergency situations, using a structured clinical approach. Initiate appropriate treatments or referrals based on clinical findings and guidelines. Perform and interpret diagnostic tests to inform decision-making. Use clinical frameworks (e.g. NICE) to guide safe, effective care.
Making appropriate referrals to primary, secondary and social services care, following practice (and/or locally agreed) pathways and protocols.
Deliver high quality clinical care in a timely manner to patients.

Attend and participate in Practice meetings when required.
Any other delegated duties appropriate to the post
