An independent prescribing clinical pharmacist post in the PCN pharmacy team at Lichfield and Burntwood GP Network, linking the network's practices with the local hospital, community pharmacies and care homes. The core work is medicines optimisation: long-term condition and medication review clinics, minor ailment caseload, discharge reconciliation and repeat prescribing. What sets it apart is the reach into care homes and domiciliary visits alongside practice-based clinics and telephone medicines support. Fit depends on GPhC independent prescribing, MPharm and GPhC registration, and at least a year as a clinical pharmacist in general practice.

- Provide patient-facing medicines support clinics and a telephone helpline for medicine queries and concerns
- Answer medicine enquiries from GPs, practice staff, other healthcare teams and patients, with follow-up
- Review medicines most linked to unplanned admissions and readmissions and put changes in place for high-risk patients
- Reconcile medicines after hospital, intermediate care and care home transfers and maintain continuity of supply
