Job summary

NHS Jobs in Grantham is recruiting for a Practice Pharmacist to lead structured medication reviews and support patients with their medicines. You will review repeat prescriptions, provide medicines advice, and work with the practice team to promote safe, cost-effective prescribing.

You'll contribute to medicines optimisation, participate in audits, and help implement prescribing guidance while supporting patient education and shared decision-making within the GP practice.

Responsibilities

• Conducting structured medication reviews and supporting patients with their medicines.
• Reviewing and processing repeat medication requests, ensuring prescribing is safe, appropriate and cost-effective.
• Responding to dispensary and medication-related queries from patients and practice staff.
• Providing advice on medicines, side effects, interactions, adherence and appropriate use.
• Supporting medicines optimisation and promoting evidence-based prescribing.
• Identifying opportunities to improve prescribing quality, safety and value.
• Supporting the management of high-risk medicines and appropriate monitoring.
• Undertaking clinical and prescribing audits and supporting quality improvement initiatives.
• Reviewing medication following hospital discharge, outpatient appointments and changes in clinical circumstances.
• Supporting the practice with implementation of relevant prescribing guidance and recommendations.
• Working collaboratively with the practice dispensary team to ensure safe and efficient medicines management.
• Providing appropriate patient education and supporting shared decision-making.
• Contributing to clinical governance, patient safety and practice improvement.
• Undertaking any other appropriate duties within the scope of the role.
