Beacon in Skegness is seeking a motivated Pharmacist to join our coastal practice. You will lead medicines reviews, support long‑term condition management and assist GPs with complex prescribing queries in a collaborative team environment.

The role focuses on safe, effective prescribing, medicines safety initiatives, and patient education within a busy primary care setting.

Qualifikationen

• Experience with structured medication reviews (SMRs) and polypharmacy reviews.
• Strong clinical reasoning and communication skills.
• Proactive, organised team player with ability to work across primary care teams.
• Willingness to work towards Independent Prescriber status if not already held.

Aufgaben

• Lead structured medication reviews and medicines reconciliation.
• Support long-term condition management (hypertension, asthma, COPD, diabetes).
• Provide clinical advice to GPs and nursing staff on safe prescribing.
• Contribute to audits, QOF, IIF, and medicines safety initiatives.
• Educate patients about medicines and their proper use.
• Collaborate with GPs, nurses, HCAs and other colleagues to ensure safe prescribing.

We are looking for a motivated, patientcentred Pharmacist to join our friendly, forwardthinking coastal practice.

In post, you'll play a key role in medicines optimisation, longterm condition management, and supporting safe, effective prescribing.

This role is ideal for someone who thrives in a collaborative environment, enjoys problemsolving, and wants to make a meaningful impact on patient care within a supportive environment.

The Role includes:

• Medication reviews structured medication reviews (SMRs), polypharmacy reviews, and medicines reconciliation
• Longterm condition support contributing to management of hypertension, asthma, COPD, diabetes and more
• Clinical support assisting GPs with complex prescribing queries and safety monitoring
• Quality improvement contributing to audits, QOF, IIF and medicines safety initiatives
• Patient education supporting patients to understand and manage their medicines
• Collaborative working working closely with GPs, nurses, HCAs, other multi-agency colleagues, As a Practice Pharmacist, you will deliver structured medication reviews, support longterm condition management, provide clinical advice to patients, and work closely with our GPs and nursing team to ensure safe, effective prescribing.
• You will also play a key role in supporting our experienced dispensary team with medicines queries, clinical oversight and workflow safety.

Have experience in primary care, community, hospital or PCN settings

Have strong clinical reasoning and communication skills

To be a proactive, organised, supportive team player

To hold an Independent Prescriber qualification (or be willing to work towards it)

We are a bust coastal Practice, with three surgeries in the Skegness area.
