Job description

Practice Nurse with an interest in Diabetes / Diabetes Nurse

Main Duties and Responsibilities:

  1. Deliver independently manage nurse-led diabetes clinics for Type 1 and Type 2 patients.
  2. Undertake diabetes reviews, including blood glucose monitoring, foot examinations, and risk assessments.
  3. Initiate and manage insulin and oral hypoglycaemic medications in line with guidelines where required.
  4. Provide structured education and lifestyle advice, supporting self-management (e.g., DESMOND).
  5. Monitor and optimise glycaemic control and other cardiovascular risk factors.
  6. Support patients with complex diabetes needs and comorbidities.
  7. Maintain accurate patient records and use EMIS Web effectively for clinical documentation, recalls, and audits.
  8. Support practice achievement of QOF and IIF diabetes targets.
  9. Collaborate with GPs, practice nurses, pharmacists, and external specialists.
  10. Participate in clinical audits and quality improvement initiatives.
  11. Keep up to date with current diabetes guidelines and professional development.

Communication & Teamwork:

  • Work collaboratively with GPs, practice nurses, healthcare assistants, clinical pharmacists, and other team members.
  • Liaise with external agencies, including community diabetes teams, dietitians, and hospital specialists.
  • Provide advice and guidance to colleagues on diabetes-related care.

Audit, Quality & Service Development:

  • Participate in clinical audits and quality improvement initiatives.
  • Identify service development opportunities to improve diabetes outcomes within the practice.
  • Keep up to date with national guidelines (e.g., NICE) and implement evidence-based practice.
