Job Summary

The post holder is responsible for the delivery of general practice nursing care to the whole practice population. Key elements of the role are the delivery of evidence based practice for patients presenting with an immediate nursing need or a long term condition and the provision of preventative health care to the practice population. They will demonstrate safe clinical decision-making and provide expert care in the management of patients. The post holder is expected to work collaboratively with all members of the practice team to meet the needs of patients and support practice objectives, policies and procedures. The post holder will work within their scope of practice and in accordance with local policies and procedures and the NMC Code of Professional Conduct.

Main duties of the job

• Assisting in and performing routine tasks related to patient care as indicated by patient need - including but not limited to wound care, Doppler, removal of sutures/clips, urinalysis, chronic disease management, venepuncture, routine immunisations and childhood immunisations
• Following agreed clinical protocols
• Upholding the standards required for Infection Prevention and Control
• Monitoring vaccine fridge temperatures
• Maintaining clinical rooms, stocking and rotating items as required
• Ordering of vaccines to maintain stock levels, if required
• Maintaining accurate records
• Attending and participating in any practice/clinical meetings
• Undertaking home visits, if required
• Supporting income generation for the Practice as relevant to the role (e.g. QoF)

Job Description / Responsibilities

• Clinical: Assist in and perform routine tasks related to patient care as indicated by patient need including cytology, wound care / ulcer care / Doppler, removal of sutures/ clips, new patient medicals / urinalysis, travel clinic, asthma and COPD management, venepuncture, hypertension management / 24 hour ambulatory BP, diabetes management, ear syringing, routine immunizations / childhood immunisations, chaperoning and assisting patients where appropriate who are being examined by another clinician, requesting pathology tests, following clinical protocols. Maintain infection prevention and control standards, monitor vaccine fridge temperatures, maintain GP and Nurses rooms, stocking and rotating items, ensure tidiness of treatment rooms, supervise HCA, order health promotion literature, order vaccinations to maintain stock, participate in practice administration, maintain records, attend monthly practice/clinical meetings, respond to home visits if required, support QoF and income generation.
• Communication: Use sensitive communication styles to inform and obtain consent; support patients receiving bad news; accommodate diverse communication needs; advocate for patients; refer to sources of support (e.g. PALS)
• Delivering a quality service: Work within your competence and NMC code; maintain accurate records; manage workload; deliver care according to NSF, NICE guidelines and evidence-based care; assess and improve care quality; participate in governance, audits and learning from incidents; collaborate on policy implementation; support and develop the GPN team; address safeguarding and domestic violence knowledge
