A primary care network in Leicester is seeking a Nurse Associate or Practice Nurse to support chronic disease management and health promotion. The role involves routine patient care tasks, participation in the screening services, and working collaboratively within a team. Candidates must have NMC registration and a solid understanding of community healthcare. This position offers a chance to work in a supportive environment focused on delivering patient-centered care.
We are looking for a Nurse Associate or Practice Nurse to join our PCN, working alongside our PCN Practices' nursing teams to support delivery of chronic disease management, wound management and health promotion to our PCN population.
The postholder will work alongside practice nursing team colleagues and within their professional boundary to:
Initiate and participate in health promotion activities
Support delivery of evidence-based programmes of chronic disease management for the PCN population
Participate in the development of our practice nursing teams
Leicester City South Primary Care Network provides care to the 32,000 patients that make up the practices of Saffron Health, The Hedges Medical Centre, Sturdee Road, Inclusion Health Care and Leicester City Assist. We are a well-established, cohesive and supportive PCN that delivers high quality and patient-centred care in order to improve the outcomes of the populations that we serve.
Assist and perform routine nursing tasks related to patient care, such as BP checks, weight and height measurements, pulse and temperature recordings, BMI and home BP monitoring
Assist and perform routine duties in line with competency; for example, wound care, phlebotomy, urinalysis, and the taking and preparing of pathology samples
Undertake would dressing procedures following practice guidelines
Support chronic disease management including monitoring patients with hypertension, diabetes, COPD and asthma, in accordance with practice protocols, local and national guidelines
Support equitable service of chronic disease management to housebound patients, undertaking home visits
Participate in cervical screening service for eligible patients in accordance with local and national policies and guidance
Support the delivery of a health promotion programme that encourages patients to adopt a healthy lifestyle in accordance with national targets, local priorities and the individual's identified health need
Provide NHS Health Checks
Support immunisation programmes, administering vaccines in line with competency and identified on a PSD
Administering of medication, where necessary in accordance with regulatory guidelines
Administering injections in line with competency and identified on a PSD, to include Influenza, Pneumonia, Hepatitis B, Tetanus, Hydroxocobalamin
To be competent in anaphylaxis and resuscitation techniques, have knowledge of the emergency equipment and drugs available in each practice and ensure the replacing and restocking of emergency equipment after use
