A prominent healthcare provider in Exmouth is looking for an experienced Practice Nurse for maternity cover. We are looking for an experienced Practice Nurse, on a maternity cover basis, to join our friendly supportive and dynamic team in Exmouth, which is on the beautiful East Devon coast. This is a key role within the clinical team and we are looking for someone confident and competent in delivering high-quality care in a busy general practice setting.

The post holder is responsible for the delivery of general practice nursing to the whole practice population. To deliver evidence-based practice for patients presenting with a long-term condition and the provision of preventative health care to the practice population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures.
Rolle Medical Partnership provides high quality primary care under a General Medical Services Contract delivering full general practice and additional enhanced services as well as care and support for our patients with long term conditions who live within our area.
Our list size is around 27,000 and we monitor our size and workload to ensure we have adequate clinical and administrative staff to adapt accordingly. We have four surgery sites, including our dispensing surgery in Lympstone.

Keyresponsibilities
Clinical practice
Assess, plan, develop, implement andevaluate treatment programmes that promote health and well-being
Assess, plan, implement and evaluateindividual treatment plans for patients with a known long-term condition
Work with other health careprofessionals to monitor, manage and treat long-term conditions, includingnon-drug-based treatment methods using a management plan, and in line withnational and local policies and practice needs.
Review medication for therapeuticeffectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols with support fromNMP or GP.
Work with patients in order tosupport adherence to prescribed treatments
Support patients to adopt healthpromotion strategies that promote patients to live healthily, and applyprinciples of self-care
Support and manage health needs ofwomen presenting for family planning and cervical cytology consultations
Recognise, assess and refer patientspresenting with mental health needs in accordance with the NSF for MentalHealth
Implement and participate invaccination and immunisation programmes for both adults and children
Advise, support and administervaccinations where appropriate for patients travelling abroad
Promote and deliver evidence-basedcare for patients presenting with aural conditions
Meet the needs of patients presentingfor opportunistic wound care
Communication
Demonstrate sensitive communicationstyles to ensure patients are fully informed and consent to treatment
Communicate with and support patientsreceiving bad news
Communicate effectively with patientsand carers, recognising the need for alternative methods of communication toovercome different levels of understanding, cultural background and preferredways of communicating

Understand and apply legal policythat supports the identification of vulnerable and abused children and adults,being aware of statutory child/vulnerable adult health procedure and localguidance
Work within policies regarding familyviolence, vulnerable adults, substance abuse and addictive behaviour, and referas appropriate

Ensure safe storage, rotation anddisposal of vaccines and drugs.
Oversee the monitoring, stock controland documentation of controlled drug usage according to legal requirements
Support members of the nursing teamto undertake mandatory and statutory training requirements
Apply infection-control measureswithin the practice according to local and national guidelines
Apply policies that reduceenvironmental health risks, are culturally sensitive and increase access tohealthcare for all
Participate in the localimplementation strategies that are aligned to the values and culture of generalpractice
Use technology as an aid tomanagement in planning, implementation and monitoring, presenting andcommunicating information
Review and process data usingaccurate Read codes to ensure easy and accurate retrieval for monitoring andaudit processes
Manage information searches using theinternet and local library databases, for example, the retrieval of relevantinformation for patients on their condition
