Main duties of the job

This role requires the candidate to be qualified by way of suitably accredited courses; i.e diploma or degree, in relevant chronic diseases and we expect you to be able to:

  • Initiate the process of diagnosis on patients with suspected disease.
  • Manage patients newly diagnosed and ongoing healthcare using practice template protocols/NICE Guidance and local guidelines.
  • Undertake annual review of patients understanding and ability to self manage.
  • Recognise areas of disease management that may be improved by altering medication and refer to GP as appropriate.
  • Refer to other healthcare professionals as necessary
  • Maintain a disease register.
  • Audit outcomes of care against national and locals standards and initiate change as necessary.
  • Offer treatment room activities such as (for example) vaccinations, wound care, immunisations and cytology.
About us

We are based in the East Yorkshire market town of Driffield. Driffield is a market town of 13,000 people. The rural area contains a further 12,000 people.The practice has approximately 15,000 patients. These are served from the Park Surgery in Driffield and the Branch Surgery in Nafferton. We dispense NHS drugs to approximately half our patients.

The Park Surgery is a modern purpose-built medical facility which we moved into in February 2011. The Branch Surgery is in the village of Nafferton. The Practice is a partnership of 2 GPs, 7 salaried GPs, Practice Nurses, Pharmacists, Nurse Practitioners, Nursing Associates, Healthcare Assistants and about thirty support staff (receptionists, dispensers, care coordinators, administrators and managers).

We are an approved practice for vocational training of GPs and are also involved in medical training and have FY2 doctors and medical students regularly at the practice.

The practice is involved in clinical research opportunities.

We are proud to be an AccuRX ambassador and work as a total triage practice, giving patients the right care at the right time.

The practice is part of the Yorkshire and Wolds PCN.

Job responsibilities

Clinical NursingPractice

It is essentialthat the applicant has general treatment room skills and that they have aDiploma in the management of one ormore of the chronic diseases (i.e Respiratory disease, diabetes, coronary heartdisease.

Any additionalskills/competencies would be useful (such as family planning, contraception and IUD's and ring pessaries) and training can be undertaken asnecessary, dependent uponthe needs of the practice population.

1. ASSESSMENT,SCREENING AND HEALTH EDUCATION

a. Provide evidence based nursingtreatments to patients - in participation with general practitioners orindependently - to agreed protocols.

b. Cover general treatment roomresponsibilities, including assistance with minor surgery.

c. Provide general and specific healthscreening to the practice patients (within agreed protocols, with referral togeneral practitioners as necessary).

d. Administer and give advice to patientsrequiring travel and routine immunisations according to Patient GroupDirections.

e. Take responsibility for day-to-dayadministration and management of aspecific chronic disease management programme (with appropriate accreditedtraining).

f. Treatment in line with local enhanced services such as IUD, ring pessaries, wound care (complex and simple), spirometry/feno, ECG's etc.

g. To work only in accordance with theUKCC Code of Conduct.

2. PATHOLOGICALSPECIMENS AND INVESTIGATORY PROCEDURES

a. Undertakecollection of pathological specimens including intravenous blood samples,swabs, smears etc.

b. Performany investigatory procedures requested by the general practitioners, or nursepractitioner. In some circumstances instigate investigatory procedures forexample blood glucose monitoring, usingown discretion.

3. CHRONICDISEASE MANAGEMENT/LONG TERM CONDITIONS

Diabetes Mellitus

This role requires completion of asuitable accredited course; i.e diploma or degree

a.Initiates the process of diagnosis on patients withsuspected diabetes.

b.Manages patients newly diagnosed and ongoing diabetesusing practice template protocols/NICE Guidance and local guidelines.

c.Undertakes annual review of patients understanding andability to self manage.

d.Recognises areas of disease management that may beimproved by altering medication and refers to GP as appropriate.

e.Refers to GP, dietician, chiropodist and diabetes andspecialist nurses as necessary

f.Maintains a disease register.

g.Audits outcomes of care against standards and NSF forDiabetes and initiates change as necessary.

Respiratory Care

This requires accreditedtraining; i.e. Diploma level

a.Manages newly diagnosed patients with asthma.

b.Maintains a disease register.

c.Works within agreed local/BTS guidelines and patientgroup directions to manage patients with asthma, aiding diagnosis, reviewinginhaler technique and advising on treatment changes.

Cardiovascular Disease

a.Develop and maintain practice register for secondaryprevention of CHD.

b.Works with patients to set action plans for changes oflifestyle.

c.Works within agreed protocols and templates whenmanaging risk factors to enable patients to manage own disease.

d.Provides psychological support by discussinginterventions and treatments.

e.Recognises areas of disease management that may beimproved by altering medication and refers to GP as appropriate.

4. Quality andAssurance

a.Work with the Partners and Practice Manager to support thePractice strategy and forward view.

b.Working with the Nursing and HCA team members, participate inquality and improvements where identified.

c.Participate in clinical supervision both as supervisor andsupervisee where appropriate.

d.Monitor the effectiveness or their own clinical practice throughquality assurance strategies such as the use of peer audit and review.

e.To be aware of ones own clinical responsibilities appropriately(ensuring safe practice and the task is within the scope of practice of theindividual).

f.Be aware and proactive of own responsibilities in relation toClinical Governance, health & safety and infection control.

g.Maintain professional registration.

h.Work within the latest NMC Code of Professional Conduct.

i.Work collaboratively with colleagues both internally andexternally, promoting effectivecommunication, relationships and team working within the practice.

j.Partake in ensuring that practice policies are fully implementedand maintained.

k.Participate in audits and inspections as required.

l.Demonstrate due regard for the safeguarding of children andvulnerable adults.

5. ADMINISTRATIVEAND PROFESSIONAL RESPONSIBILITIES

a. To ensurethat accurate notes of all consultations and treatments are recorded in thepatient record.

b. To ensureaccurate completion of all necessary documentation associated with patienthealth care and registration with the practice.

c. To ensurecollection and maintenance of statistical information required for regular andad hoc reports.

d. To attendand participate in any practice or team meetings as required.

6. OTHERDUTIES

a. Co-operateand participate as required in any research and audit projects within thepractice.

b. Participatein the education and training of students of all disciplines and the inductionof all members of practice staff.

c. Maintaineffective liaison with other agencies and staff concerned with patient care andwith all other disciplines within the practice.

d. Maintaincontinued education by attendance at courses and study days as deemed useful ornecessary for professional development.

e. Participationin extended hours rota as required

f. Ensureawareness of responsibilities in relation to Clinical Governance, health &safety and infection control.

g. Any otherduty as shall reasonably be required.
