A healthcare provider in Brigg is looking for a highly motivated Long-Term Conditions Practice Nurse. You will deliver high-quality care to patients with chronic conditions like Asthma and Diabetes. Responsibilities include conducting patient reviews, implementing care plans, and providing patient education. Ideal candidates will have extensive experience in primary care and relevant qualifications in chronic condition management.
We are seeking a highly motivated and experienced Long-Term Conditions (LTC) Practice Nurse to join our friendly and supportive General Practice Team. This is an exciting opportunity to deliver high-quality care to patients with long-term conditions, helping them to manage their health and improve their quality of life.
As part of our team, you will play a key role in supporting patients with conditions such as Asthma, COPD, Diabetes and CVD, providing expert clinical care, education, and monitoring.
Key Responsibilities
Riverside Brigg is a purpose built practice in the centre of Brigg with four other sites in surrounding areas. Five GP partners care for nearly 24,500 patients; we are a Training Practice, host medical students and nurses and also offer work placements. We are a dispensing practice with a pharmacy attached at both sites in Brigg and Broughton. The partners have specialist interests in Cardiology, Dermatology, Diabetes, Minor Surgery, Respiratory and Substance Misuse. We employ an ANP with an interest in paediatrics. The Urgent Care Team manages on the day demand offering emergency appointments and telephone advice. Practice nurses manage long term conditions within the surgery and are supported by two Nurse Associates and six HCAs. The two practice Pharmacists manage prescribing and medicines optimisation. The clinical team is supported by a multi-professional administration team.
Main Duties and Responsibilities of Post
To be competent and confident in managing people with long term health conditions
To ensure practice guidelines relating to Asthma, COPD, Diabetes and CVD are up to date, discuss any changes and implement in collaboration with Clinical Leads.
To audit each disease area is moving toward target, highlight and action any problem areas discuss any issues with Clinical Leads.
To proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition.
To diagnose and manage long term conditions, integrating both drug- and non-drug-based treatment methods into a management plan (as appropriate).
To review and re-authorise medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
To work with patients to support compliance with, and adherence to, prescribed treatments, promoting self-management.
To provide information and advice on prescribed or over-the-counter medication, medication regimes, side-effects and interactions to long term conditions.
To provide assessment, screening, treatment, monitoring and health education advice to long term conditions.
To utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
To prioritise own workload and ensure effective time-management strategies are implemented.
To produce accurate, contemporaneous and complete records of patient consultations and treatments, consistent with legislation, policies and procedures.
To undertake home visits, as appropriate.
To attend and participate in Practice meetings.
To recognise and work within own competence and professional code of conduct as regulated by the NMC.
To deliver care according to NICE guidelines and other relevant evidence-based care.
