One Medical Group in Aberdeen is seeking an experienced Practice Nurse on a fixed-term contract (12 months) to cover MAT leave. The role involves long-term condition management, health promotion, immunisations and cervical cytology within a patient-centred primary care setting.

You’ll work with multidisciplinary teams to deliver safe, effective, evidence-based care, supporting prevention and early intervention for patients’ health and wellbeing.

Responsabilités

• Long-Term Condition Management: Assessing, planning and reviewing individualised care for patients with long-term conditions, supporting ongoing monitoring and effective self-management.
• Health Promotion & Prevention: Providing health education, lifestyle advice, screening and preventative interventions to help patients maintain and improve their health.
• Vaccinations & Immunisations: Delivering adult and childhood vaccination and immunisation programmes, as well as travel health advice and vaccinations where appropriate.
• Cervical Cytology & Women’s Health: Undertaking cervical cytology consultations in line with the national cervical screening programme and providing contraception and sexual health advice within your competencies.
• Wound Care: Assessing and managing uncomplicated wounds using evidence-based practice.
• Patient Assessment: Assessing and prioritising patients presenting with routine, urgent or emergency needs and responding appropriately within your scope of practice.
• Safeguarding: Recognising and responding appropriately to safeguarding concerns involving children and vulnerable adults.
• Quality & Governance: Maintaining accurate clinical records and contributing to QOF, enhanced services, clinical audit, incident reviews, peer review and continuous improvement.
• Collaborative Working: Working closely with colleagues across the multidisciplinary team to provide safe, coordinated and patient-centred care.
