Job description

Practice Nurse

Auchtermuchty Health Centre

An exciting opportunity has arisen at Auchtermuchty Health Centre for a Practice Nurse with experience in chronic disease management.

We are seeking an individual skilled in the management of one or more long-term health conditions, which could include:

• Diabetes
• Hypertension
• Heart failure
• COPD
• Asthma

Hours and working patterns are flexible to suit both parties and pay will be commensurate with skill and experience.

Key Responsibilities:
• Long-term management of patients with chronic disease through clinic reviews.
• Reviewing patient health questionnaires and test results.
• Identifying and managing patients who would benefit from optimisation of their health through education, lifestyle change and medication.
• Ideal candidate will be responsible for some stock ordering and replenishing.
• Core nursing skills of wound care, phlebotomy, ECG taking, injections, cervical screening are essential (though it is important to note that this is not a large part of the role as many of these core services are now provided by our CTAC colleagues)

• Semi-rural GMS practice with approx. 6000 patients, using EMIS and DOCMAN IT systems.
