Practice Nurse - General Practice - Part-time

An opportunity has arisen for a new Practice Nurse to join our friendly team.

We are looking for a practice nurse to work 3 full days a week.

Main duties of the job

The successful applicant would be expected to provide nursing services to the practice population with evidence based, quality focused care. To assess, plan, implement and evaluate care. To ensure clear channels of communication exist between clinical, administrative and community teams. To project a positive and friendly image to our patients and other professionals. To lead infection control policy & process within our practice and ensure the practice is compliant with CQC IPC requirements.

About us

Grosvenor House Surgery is located in West Ealing, W13 9BE and is a member of the South Central Ealing Primary Care Network (SCE PCN). The practice has a list size of 5500 patients. Staff comprise of 4 GPs, 2 Practice nurses, 1 trainee nurse, 1 GP assistant, pharmacy clerk and 7 Reception and administrative staff. The practice uses SystmOne.

South Central Ealing PCN provides additional clinical and administrative staff to support the practice (Clinical Pharmacy team, First Contact Physiotherapist, Mental Health Practitioner, Paramedic home visiting service and Social Prescriber.

Job responsibilities

Administering vaccinations and immunisations in line with national and practice guidelines

  • Cervical cytology
  • Managing and monitoring patients with chronic diseases (e.g. asthma, COPD, diabetes, hypertension) in line with national and local guidelines
  • Carrying out home visits for housebound patients
  • Ensuring accurate and timely clinical record keeping and documentation
  • Conducting contraception reviews and coil checks
  • Carrying new patient and NHS health checks
  • Wound care and management including dressing, removal of sutures and clips
  • Participating in infection control, cold chain maintenance, ensuring fridge temperatures are regularly monitored and maintained
  • Supporting patients with lifestyle advice and self-care management
  • Advising patients on health care, minor ailments, health promotion, and illness prevention
  • Contributing to achievement of QOF (Quality and Outcomes Framework) and local enhanced services achievement and clinical audits
  • Preparing and maintaining clinical rooms and equipment before, during and after patient care activities, ensuring a clean and safe environment
  • Ensuring emergency medication and resuscitation equipment is adequately stocked and fit for purpose
  • Ensuring proper handling and dispatch of laboratory specimens including blood and urine samples
  • Participating in practice meetings and contributing to our aim for continuous improvement
  • Supporting the training and mentoring of junior staff members and HCAs
Person Specification
Experience
  • Experience of working within a nursing team in primary care
  • Excellent communication and interpersonal skills, and be confident to deal with patients, families and carers
  • Ability to work independently and as part of a multidisciplinary team, using initiative within the level of competence for the position
  • Commitment to patient confidentiality and data protection
  • Accuracy and a strong desire for attention to detail
  • Commitment to continuing professional development
  • Working knowledge of SystmOne clinical system
  • Stable career history showing clear commitment and progression
Qualifications
  • Registered General Nurse with current NMC registration
  • Qualified & competent in cervical cytology, childhood & adult immunisations, chronic disease management and phlebotomy
