A healthcare provider in Leeds is looking for a part-time practice nurse to join their supportive nursing team. The role involves managing chronic diseases, providing vaccinations, and working collaboratively with GPs and other healthcare staff. Ideal candidates have experience in primary care and a willingness to learn about respiratory care. Full responsibilities include treatment room tasks, home visits, and some telephonic reviews.
We are looking for a part time practice nurse to work 19 hours over Monday to Wednesday with a desire and willingness to obtain respiratory qualifications to join our experienced and supportive nursing team.
We would ideally like you to have experience in primary care and of working autonomously providing management care in long term conditions , particularly in respiratory, or a willingness and desire to learn and develop in this area.
Our practice nurses undertake the full range of treatment room and chronic disease management nursing, working closely alongside the wider practice team which includes GPs, HCA's, Nurse Associate, Phlebotomists, First Contact Practitioners, Clinical Pharmacists and our Social Prescribing Team.
We also see patients in their homes or care homes for vaccinations and reviewing their long term conditions.
We are a large practice across 2 sites in Garforth and Barwick in Elmet with a population size of over 16000 patients
We are a teaching practice and regularly have Registrars, Medical Students and Nursing Students.
We have 10 GP's, an ANP, Practice Matron and a nursing team of 5 Nurses, 3 HCA's and a Phlebotomist.
Our nursing team undertake the full range of treatment room including wound care, immunisations and vaccinations, travel advice, cytology, phlebotomy and disease management both in practice and in patients homes. We also undertake reviews via the telephone.
