Job summary

Living Well Partnership is seeking a capable Practice Nurse to deliver high-quality nursing care, health assessments and treatments within a multidisciplinary team. You will educate patients, support chronic disease management and promote healthy lifestyles in line with NMC standards.

You will collaborate with GPs and nurses to optimise patient outcomes, maintain clinical records, and participate in policy and service development initiatives.

Qualifications

  • Essential: NMC registered nurse with practice nurse experience.
  • Experience with chronic disease management is desirable.
  • Strong communication and teamwork abilities required.

Responsibilities

  • Conduct patient assessments including history taking and vital signs.
  • Provide immunisations and cervical screening where applicable.
  • Deliver wound care, ECGs, injections and related procedures.
  • Support long-term condition management (diabetes, COPD, etc.).
  • Maintain accurate clinical records using the practice’s electronic system.
  • Order vaccines, drugs and laboratory supplies as needed.
  • Participate in practice meetings and service development initiatives.

Job summary

As a Practice Nurse, you will provide high-quality nursing care, health assessments and treatments to patients, working closely with GPs and other healthcare professionals. You will play an important role in patient education, chronic disease management and health promotion, while working within the NMC Code of Conduct and the Partnerships policies and procedures

Main duties of the job
  • Conduct patient assessments including history taking physical examinations and vital signs monitoring relevant to role
  • Administer treatments and procedures such as wound care ECGs Doppler assessments blood pressure checks and injections
  • Provide immunisations including child and travel vaccinations and perform cervical screening
  • Provide contraceptive care and advice
  • Support the management of patients with chronic conditions such as diabetes asthma COPD and cardiovascular disease
  • Monitor and review treatment plans ensuring they align with best practices and patient needs.
  • Collaborate with GPs and other healthcare professionals to optimise patient care
  • Promote healthy lifestyles and offer guidance on diet exercise and smoking cessation
  • Maintain accurate and detailed patient records, ensuring all treatments and consultations are documented in the practices computerised system
  • Monitor and manage medical supplies, including the ordering of vaccines drugs and laboratory materials
  • Participate in practice meetings, contributing to discussions on patient care practice development and policy updates
  • Work as part of a multidisciplinary team, supporting colleagues and sharing expertise to improve patient outcomes
  • Maintain a Personal Learning Plan to ensure compliance with revalidation requirements and NMC standards

Job responsibilities
  • Conduct patient assessments, including history taking, physical examinations and vital signs monitoring appropriate to your role.
  • Deliver a range of treatments and procedures including wound care, ECGs, Doppler assessments, blood pressure monitoring and injections.
  • Provide immunisations, including childhood and travel vaccinations, in line with national guidance.
  • Undertake cervical screening and contribute to womens health services.
  • Provide contraceptive advice, reviews and ongoing care within your competence.
  • Support the management of patients with long-term conditions such as diabetes, asthma, COPD and cardiovascular disease.
  • Monitor and review treatment plans, ensuring care remains evidence-based and responsive to patient needs.
  • Work collaboratively with GPs, pharmacists, specialist nurses and the wider multidisciplinary team to optimise patient outcomes.
  • Promote health and wellbeing and support patients with lifestyle changes including smoking cessation, healthy eating and physical activity.
  • Maintain accurate and timely clinical records using the practices electronic patient record system.
  • Contribute to the management of clinical stock, vaccines and medical supplies as required.
  • Participate in practice meetings, clinical discussions, audits and service development initiatives.
  • Share knowledge, support colleagues and contribute positively to the ongoing development of the nursing team.
  • Maintain a Personal Development Plan and engage in ongoing learning to support NMC revalidation and professional development.

  • Computer literacy is essential.
  • Necessary training and support in clinical skills and clinical documentation systems will be provided.
  • Experience in Chronic respiratory disease management is desirable.
