Do you have experience as a Practice Nurse (Chronic Disease Specialist) in a Primary Care setting? If so, keep reading! A GP surgery in Plymouth is on the lookout for an experienced Practice Nurse to join their friendly, hard-working team on a permanent basis. The Practice Nurse in question MUST have a speciality in Chronic Disease Management, such as Diabetes, COPD and Asthma. The surgery in question is looking for a passionate Practice Nurse for up to full-time hours (with the minimum being 30 hours). The surgery is happy to be flexible with shift patterns for the correct candidate, and these can be discussed to suit both yourself and the surgery.
The ideal candidate will:

1. Have prior GP experience.
2. Have recognised qualifications/training in Chronic Disease Management (Diabetes, COPD, and Asthma).
3. Feel comfortable running their own clinics in Cytology, Baby and Travel Immunisations, Vaccinations, Dressing and wound care, Family planning, and more.
4. Have experience with System One.

In return, the surgery can provide a pay of around £ (dependent on experience), a generous annual leave package plus bank holidays, NHS Pension, free parking, and more! The successful candidate would be joining a large and diverse team including nurses, paramedics, pharmacists and more. The practice is part of a larger PCN working through a patient list of 43,000.
