Practice Nurse, Chronic Disease - Crewkerne, Somerset

Are you an experienced Practice Nurse, specialising in Chronic Disease (e.g., respiratory / COPD / Asthma)? Looking for a new challenge in Primary Care?

Symphony Healthcare Services (SHS) has an exciting opportunity to join Crewkerne Health Centre in Somerset.

This is a permanent, part-time post of 31.5 hours per week.

The annual salary range for this post is £39,146.36 - £41,851.01 per annum (pro rata for part-time hours, depending on experience).

Main duties of the job

The main duties of a SHS Chronic Disease Practice Nurse are:

  • Manage a designated caseload of patients with chronic conditions, visiting them in their homes or consulting with them at the practice.
  • Work within the NMC Code.
  • Mentor junior staff, supporting their development.
  • Develop nursing services for the practice and through SHS.
  • Support clinical supervision within the practice.
  • Collaborate with the practice and Lead Nurse to develop the clinical skill mix.
  • Promote SHS values and act as a role model.
  • Implement the SHS Nursing Strategy as defined by the Nursing and AHP Director and the Lead Nurses forum.
  • Ensure high patient involvement and satisfaction, including liaising with the practice PPG.

Qualifications
