Job Summary

We are seeking a dedicated and compassionate Practice Nurse to join our dynamic team within a General Practice (GP) setting. The successful candidate will play a vital role in delivering high-quality, patient-centered care across a wide range of clinical responsibilities. This position involves working closely with GPs, healthcare assistants, and other members of the multidisciplinary team to support health promotion, disease prevention, and chronic disease management.

As a Practice Nurse, you will be responsible for providing skilled nursing care and clinical services such as immunisations, wound care, cervical screening, and health assessments. You will also support patients with long-term conditions like diabetes, asthma, and hypertension through monitoring and education. • Chronic Disease Management
• Monitoring and supporting patients with long-term conditions such as:

• Conducting reviews and care planning
• Educating patients on self-management and lifestyle modifications
• Providing advice and education on smoking cessation, weight management, physical activity, alcohol use, and sexual health
• Running health screening and NHS Health Check clinics
• Administering routine and travel vaccinations for adults and children
• Providing seasonal flu and COVID-19 vaccinations
• Ensuring vaccine storage and cold chain compliance
• Performing smear tests in line with national screening programmes
