Job summary

Uplands Medical Practice is seeking a part-time practice nurse for a one-year fixed-term post, three days a week within the nursing team of our GP practice. Core duties include chronic disease reviews, immunisations, cervical screening and preventative care.

Experience in COPD, asthma and diabetes is highly valued. You must hold current RGN registration with the NMC and be committed to patient self-management, quality improvement and collaborative care with GPs, HCAs and pharmacists.

Qualifications

• RGN with current NMC registration.
• Evidence of ongoing professional development.
• Experience delivering primary care nursing in a GP setting.
• Ability to work as part of a multidisciplinary team.

Responsibilities

• Deliver practice nurse services within agreed competencies and NMC standards
• Undertake chronic disease reviews for diabetes, asthma and COPD
• Conduct annual reviews and care planning for patients with long-term conditions
• Administer childhood, adult and seasonal vaccinations in line with national guidance
• Undertake cervical cytology screening
• Provide lifestyle advice on smoking cessation, weight management and healthy living
• Carry out blood pressure monitoring, ECGs, venepuncture and other nursing procedures
• Support infection prevention and control processes
• Maintain accurate patient records on EMIS Web
• Participate in clinical audits and quality improvement initiatives
• Support the achievement of QOF targets
• Work collaboratively with GPs, HCAs, pharmacists and the wider multidisciplinary team

The practice is particularly keen on experience managing COPD, asthma and diabetes, with an emphasis on helping patients self-manage long-term conditions. Fit depends on current RGN registration with the NMC.
