A primary care provider is seeking an experienced Practice Nurse in Welwyn Garden City. The successful candidate will support healthcare improvement, provide treatment room duties, and collaborate with GPs and the nursing team. This permanent position requires NMC registration, with responsibilities ranging from ECGs to chronic disease management and health promotions. We have an exciting opportunity for an experienced and enthusiastic Practice Nurse to join our multi-disciplinary primary care team to improve the health and wellbeing of our patients .
Although working independently, you will work alongside our GPs, Pharmacists and Nursing Team. The succesful candidate will provide all teatment room duties and support with CQC compliance.
This is a permanent position up to 32 hours. In addition you will be required to take part in the Extended Access rota on Saturday mornings and annual vaccination clinics.
To provide nursing services to a defined practice populationwith the assistance of, and in collaboration with, the Nursing Team and GPcolleagues. Clinical duties to include: ECGs, wound care (leg ulcer level 1),BPs, Ear irrigation, Injections, cervical screening, chronic disease managementand care planning and other clinical duties as requested within capability. Tocontribute in the development of services provided by the Practice and toparticipate in clinical audit. To workwith the GPs to develop and implement the Practice Development Plan, and tomeet key performance indicators within Quality and Outcomes Framework (QOF) andEnhanced Commissioning Framework (ECF). You will be expected to work within heNMC Code of Conduct and to agreed practice standards and protocols.
The Garden City Practices is committed to development of its staff and is an accredited training practice. Our clinical team consists of 6 GP partners, 2 ANPs and a Nursing Associate as well as clinical support via the PCN Additional Roles Reimbursement Scheme (ARRS)
We are based 5 minutes walk for the town centre, with onsite parking, and are an active member of the WGC PCN with our neighbouring practices and part of the local Federation, Ephedra.
There is an experienced, friendly team of administrative staff including the Practice Manager, Deputy Practice Manager, Reception Manager & Team, Senior Administrator and Secretaries.
Our list size is growing and currently we have around 12 500 patients.
To be professionally accountable and responsible for all aspects of your own work, including the management of patients in your care, always practising in accordance with the NMC Code of Professional Conduct.
To maintain a professional portfolio which reflects personal development and to attend clinical supervision.
To be responsible for maintaining competency to practices through ones own continuous personal and professional development activities, To maintain an annual professional development plan through appraisal, in accordance with the CCG. Performance Development Scheme and that is relevant to the practice development plan.
To undertake the measurement and evaluation of your work using audit (such as cervical smear adequacy), outcome measures and evidence-based practice projects.
To carry out other duties commensurate with the grade as directed by doctors or management.
