We are looking for a practice nurse preferably with experience in Chronic Disease Management to join our friendly nursing team.

Main duties of the job

The role would include Child Immunisation, Cervical Cytology, Travel Health, Wound Care, Health Promotion, Patient Education and Chronic Disease Management if applicable.

We are a 10,500 patient practice situated in the leafy suburbs of South Buckinghamshire. We have a great friendly team of 2 GP Partners, 1 Salaried GP, 2 long term Locums, 3 Physicians Associates, 4 Nurses 1 HCA, 3 Pharmacists, 1 Social prescriber, 1 First Contact Practitioner and 14 very supportive Admin Staff.

Job responsibilities

To provide a high quality care service based on the needs of the practice population including treatment, screening, preventative care and patient education.

Person Specification

Qualifications

• Registered General Nurse minimum 2 years in post.
• Qualified in Phlebotomy

Experience

• Experience of working within a nursing team.
• Evidence of appropriate knowledge base.
• Willingness to learn.
• Experience of working in General Practice.
• Computer experience using EMIS.
