A leading healthcare provider is seeking an enthusiastic Practice Nurse to join their dedicated team at Whitburn Surgery. Whitburn Surgery is run by South Tyneside Health Collaboration - a federation of South Tyneside GP Practices.
There may be a need to work across our other sites based in South Tyneside.

The post holder will be responsible for implementing processes for the effective management of patients with long-term conditions using evidence-based practice, including care for elderly and housebound patients.
Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.
The Practice Nurse will be responsible for clinical areas such as health promotion, chronic disease management, and mentoring staff. They will actively support the practice management team in reviewing and delivering clinical policies and procedures.
Whitburn Surgery will ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organization to provide advice and support on a day-to-day basis.

Salary: £38,032 to £45,965 per year (pro-rata), depending on experience.
