Job responsibilities

Main Purpose of Job:

The post holder isresponsible for the delivery of basic Practice nursing services and care to thePractice population. Supported by senior nurses within the Practice, they willdeliver care within the boundaries of their role, focusing upon supportingpatients to be healthy, monitoring of long-term conditions, health preventionand screening activities. They will work collaboratively with the generalPractice team to meet the needs of patients, supporting the delivery of policyand procedures, and providing nurse leadership as required.

Maintasks of job:

Clinical Practice:

1)Assess, plan,develop, implement and evaluate programmes to promote health and well-being,and prevent adverse effects on health and well-being.

2)Implement andevaluate individual treatment plans for patients with a known long-termcondition.

3)Identify, andmanage as appropriate, treatment plans for patients at risk of developing along-term condition.

4)Prioritise healthproblems and intervene appropriately to assist the patient in complex, urgentor emergency situations, including initiation of effective emergency care.

5)Support patientsto adopt health promotion strategies that encourage patients to live healthily,and apply principles of self-care.

6)Deliveropportunistic health promotion using opportunities such as new-patientmedicals.

7)Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side effects and interactions.

8)Support patientsto adopt health promotion strategies that promote patients to live healthily,and encourage principles of self-care.

9)Assess and carefor patients presenting with uncomplicated wounds.

10)Support andadvise women requesting information relating to family planning needs.

11)Support andmanage health needs of women presenting for cervical cytology consultations.

12)Recognise, assessand refer patients presenting with mental health needs in accordance with theNational Service Framework (NSF) for Mental Health.

13)Implement andparticipate in vaccination and immunisation programmes for both adults andchildren.

14)Advise, supportand, where appropriate, administer vaccinations for patients travelling abroad.

15)Promote anddeliver evidence-based care for patients presenting with aural conditions.

16)Assist seniorpractitioners in providing minor-surgery sessions.

Communication:

1)Utilise anddemonstrate sensitive communication styles, to ensure patients are fullyinformed and consent to treatment.
