Duties:

The post holder will be responsible for implementing processes for effective management of patients with long-term conditions, including elderly and housebound patients, using evidence-based practices. As a senior member of the multidisciplinary team, the Practice Nurse will ensure effective delivery of nursing services to the patient population. Responsibilities include:

  • Supporting infection prevention and control initiatives, including audits and training
  • Contributing to the annual Infection Prevention Control (IPC) statement
  • Developing and implementing care management programs for frail and housebound patients
  • Assessing patient needs to ensure appropriate healthcare provision
  • Developing health promotion and wellbeing programs
  • Implementing and evaluating treatment plans for chronic disease patients
  • Supporting patients at risk of long-term conditions to prevent adverse health effects
  • Providing routine nursing care in accordance with clinical evidence, NICE, and NSF guidelines
  • Diagnosing and managing acute and chronic conditions
  • Prescribing medications within PGDs and scope of practice
  • Reviewing medications to improve compliance
  • Guiding patients on medication use and side effects
  • Coordinating travel medicine services
  • Managing pathology requests and results
  • Overseeing chronic disease clinics and referring patients to specialists as needed
  • Maintaining accurate clinical records and ensuring effective use of read codes
  • Managing chronic disease registers
  • Participating in well woman and well man clinics
  • Prioritizing health issues and intervening appropriately
  • Supporting the team in clinical emergencies
  • Assessing and referring patients with mental health needs
  • Implementing vaccination programs for adults and children
  • Supporting patients with medication management
  • Liaising with external agencies to support vulnerable patients
  • Delegating clinical responsibilities appropriately
  • Supporting safeguarding policies and procedures

Training requirements include:

  • Immunisation and Vaccination
  • Cervical Cytology
  • Chronic Disease Management (e.g., asthma, COPD, diabetes, hypertension)
  • Travel Health
  • Wound Care and Management
  • Venipuncture

Qualifications:

  • Registered Nurse with the Nursing and Midwifery Council
  • At least 2 years of practice nursing experience
