Job Overview:

As a Practice Nurse, you will be responsible for providing high-quality nursing services to patients, including immunizations, wound care, and chronic disease management. You will work closely with the GP team to deliver outstanding healthcare and promote patient well-being.

Key Responsibilities:

• Provide direct clinical care including vaccinations, health checks, and chronic disease management.
• Collaborate with GPs to develop and implement patient care plans.
• Maintain accurate patient records in compliance with regulations.
• Support health promotion and disease prevention initiatives.

Qualifications:

• Registered Nurse with a current NMC registration.
• Experience in a primary care or GP setting is preferred.
• Strong communication and teamwork skills.
