Qualifications

• NMC-registered Nurse with primary care experience.
• Up-to-date immunisation training desirable.
• Confident in chronic disease management and treatment room-work.
• Strong patient communication and clinical reasoning.

Responsibilities

• Conduct chronic disease reviews for asthma, COPD, diabetes, hypertension.
• Perform wound care, dressings and treatment room duties.
• Carry out immunisations where trained (adult and children).
• Undertake smear tests (cytology-trained nurses).
• Complete NHS Health Checks and lifestyle assessments.
• Manage results, recalls and routine follow-up tasks.
• Document clearly in the clinical system.

Requirements
