Job summary

Epsom and St Helier University Hospitals NHS Trust is seeking a Practice Nurse to join our primary care team. You will promote health, assess needs, plan and deliver nursing care, and work within the Primary Healthcare Team to improve service delivery.

Responsibilities include vaccination, wound management, phlebotomy, ECGs, chronic disease support, and patient education. You will maintain high standards, record patient data, and collaborate with GP partners and other services while upholding

  • Adhere to the NMC Code of Conduct and Scope of Professional Practice.
  • Assess, plan, implement, monitor and evaluate clinical care in partnership with patients and families.
  • Provide nursing advice and be a resource of best practice for the team.
  • Chaperone patients when required and maintain confidentiality of patient information.

  • Provide vaccination and monitor vaccine storage and handling.
  • Assist with wound care, phlebotomy, ECGs and minor procedures as competency allows.

Job description

Practice Nurse Grade: According to experience Accountable to: GP Partners, reporting to Practice Manager JOB PURPOSE To promote the mental, physical and emotional health of the Practice population To assess the health needs of the Practice population To provide and maintain a high standard of care for the Practice population To be an effective member of the Primary Healthcare Team and to link with other organisations and disciplines in the development of services DIMENSIONS Service development Clinical Practice development Clinical practice 1. Service development To work collaboratively with other members of the nursing team to influence and implement policy and change in the Practice To maintain an awareness of changing services needs within the scope of services in Primary Care, the Trust and Social Services 2. Management To be responsible for the effective use of resources within the sphere of responsibility To ensure the maintenance of accurate and up to date manual and electronic records, as well as collecting and recording activity monitoring data, care aims and other contractual requirements 3. Clinical To adhere to the NMC Code of Conduct and Scope of Professional Practice To further develop practice through continuing education which will benefit service delivery and meet educational requirements To provide nursing advice and to be a resource of best practice for the team To assess, plan, implement, monitor and evaluate clinical work carried out to ensure effective care delivery in the partnership with clients and families/carers To promote and advise on healthy lifestyles To provide treatment room duties in line with competency e.g. Suture/clip removal, Basic & complex wound care, ECG. Phlebotomy, Urinalysis To give vaccination as appropriate under patient group directions and to be responsible for the ordering of vaccines and their correct storage To assist with monitoring and support of the management of patients with long-term conditions / to chronic diseases, using a holistic patient centred approach To assist with the cervical screening programme To administer prescribed drug therapy and ensure safe custody of drugs To assist with Warfarin monitoring To assist in the provision of minor surgery, coil and implant fittings To assist with the co-ordination of Infection Control policy and audit. To Chaperone patients when required To provide training and supervision to junior members of nursing staff To ensure confidentiality on all matters relating to patients and information obtained during the course of employment To participate in the Practice/Trust/s staff development and performance management and supervision systems OTHER DUTIES To undertake such other duties as may be reasonably required from time to time as are consistent with the responsibility and scale of the post To have responsibility for the health, safety and welfare of self and others in the working environment, to follow safe working practice and to comply at all times with the Trust/Practice health and safety policy and procedures.
