Job summary

Allscreens Nationwide Ltd is seeking a Practice Nurse to work in General Practice within the Central Halifax Primary Care Network. You will deliver high-quality, patient-centred care, perform routine nursing duties, vaccinations, wound care and clinical observations, and support CPD and professional development.

You will join a collaborative nursing team, provide supervision to HCAs and students, maintain accurate records, and contribute to QOF and quality improvement initiatives in line with

Responsibilities

  • Delivery of high quality, compassionate care whilst undertaking specific clinical and care tasks, with a focus on promoting good health and independence.
  • Provide high-quality holistic and person-centred care to individuals.
  • Provide routine care to patients as required in accordance with clinical based evidence, NICE Guidelines etc
  • Work as part of larger nursing team to support colleagues
  • Improve safety and quality of care at every opportunity
  • Develop own clinical competence, leadership, and reflective practice skills within the workplace with Clinical Lead
  • Provide support and supervision to other colleagues within the nursing team; Healthcare Assistants, Phlebotomists, Students etc
  • Maintain accurate and contemporaneous patient health records ensuring SNOMED coding
  • Conduct Long Term Condition Reviews—Diabetes with COPD & Asthma as required
  • Provide wound care, dressings and other clinical tasks
  • Perform and record clinical observations such as BP, temp, pulse
  • Provide vaccinations including Flu, COVID & child immunisations
  • Provide other services such as venipuncture, Cervical Screening, NHS Health Checks
  • Undertake additional training to provide other clinical tasks
  • Enhance own performance through CPD and share knowledge
  • Care for individuals with Dementia, Mental Health conditions, Learning Disabilities
  • Advise patients on general healthcare and promote self-management
  • Communicate with colleagues across the Practice and PCN and attend meetings
  • Access regular clinical supervision with line manager
  • Contribute to clinical governance
  • Maintain a clean, tidy working area
  • Assist nursing team with stock ordering and rotation to minimise waste
  • Be an accountable professional
  • Support delivery of QOF and quality improvement initiatives
  • Assist with COVID-19 tasks
  • Undertake tasks consistent with the level of the post and scope of the role

Job description

The Practice Nurse works alongside a team within General Practice and within the Central Halifax Primary Care Network (PCN).

Key Responsibilities
  • Delivery of high quality, compassionate care whilst undertaking specific clinical and care tasks, with a focus on promoting good health and independence.
  • To provide high-quality holistic and person-centred care to individuals
  • To provide routine care to patients as required in accordance with clinical based evidence, NICE Guidelines etc
  • Work as part of larger nursing team to support colleagues
  • Improve safety and quality of care at every opportunity
  • Work with Clinical Lead to take responsibility for developing own clinical competence, leadership, and reflective practice skills within the workplace
  • Provide support and supervision to other colleagues within the nursing team; Healthcare Assistants, Phlebotomists, Students etc, and those on learning assignments/placements as required.
  • Maintain accurate and contemporaneous patient health records ensuring that clinical data is appropriately recorded with SNOMED codes
  • To conduct Long Term Condition Reviews- focusing on Diabetes, with additional COPD & Asthma as required
  • To provide wound care, dressings and other clinical tasks as required
  • Perform and record clinical observations such as blood pressure, temperature, and pulse etc.
  • Provide vaccinations including- Flu, COVID & child immunisations and other relevant clinical tasks as required by the Practice and PCN in line with the competencies of the role
  • Provide other services such as venipuncture, Cervical Screening, NHS Health Checks
  • Undertake additional training where required to provide other relevant clinical tasks as required by the Practice and PCN, in line with the competencies of the role.
  • Enhance own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service
  • Care for individuals with Dementia, Mental Health conditions, and Learning Disabilities
  • Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services
  • Communicate proactively and effectively with all colleagues across the Practice and PCN, attending and contributing to meetings as required
  • Work with your line manager to access regular clinical supervision
  • Contribute to and embrace the spectrum of clinical governance
  • Maintain a clean, tidy, effective working area
  • To assist the nursing team with stock ordering and maintaining rotation of stock to minimise waste
In addition to the primary responsibilities, the Practice Nurse may be requested to:
  • Be an accountable professional
  • Support delivery of QOF, incentive schemes, quality improvement projects, audits and other quality or cost effectiveness initiatives enhancing service delivery and patient care
  • Assist with COVID-19 tasks as required
  • Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.
  • Duties may vary from time to time without changing the general character of the post or the level of responsibility.
  • To have a good understanding of practice protocols and policies
  • Undertake all mandatory training and induction programmes
  • To act as a chaperone
