A healthcare organization in Plymouth is seeking an experienced Practice Nurse to manage chronic disease care and conduct health promotion clinics. Ideal candidates will be UK Registered Nurses with at least 2 years of experience in a general practice setting and skills in cervical cytology and immunizations. Exciting opportunity for an experienced Practice Nurse to join our friendly Plymouth PCN.
You will join a large team of Practice Nurses and Healthcare Assistants and will undertake all aspects of the practice nurse role including cervical screening, baby immunisations and chronic disease management.
We are an innovative 5 surgery PCN with a practice population of 34,000.
We are a training Practice where we encourage training and education for our own staff and we support student nurses, paramedics, physiotherapists and GP trainees.
The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policies and procedures.
Initiating and maintaining health screening and promotion clinics for identified areas of the practice population, recognising where modification of systems or referral is necessary.
Managing the nursing care of patients with chronic disease, employing personal recall systems suitable to the practice needs. This includes using the practice computer systems for input of information for recall and screening purposes.
Providing specialist care for registered patients in the following key areas: IPC, Asthma, Hypertension, Diabetes, COPD, Cervical Smears, Contraception, Baby immunisations.
Providing routine treatment in general clinics, including Wound Management, Blood tests, and ECGs.
