Job description

The Practice Nurse is a key member of the primary care team, delivering high-quality, evidence-based nursing care in a general practice setting. Working with GPs, nurse practitioners, nursing associates, healthcare assistants, and other team members, the Practice Nurse plays a vital role in managing long-term health conditions, promoting health, supporting screening programmes, and treating minor illnesses and injuries.

The role involves autonomous clinical practice within NMC scope, including chronic disease management (e.g. diabetes, asthma, COPD), wound care, immunisations, cervical cytology, family planning, and lifestyle advice. The nurse also contributes to audits, quality improvement initiatives, and service development aligned with national and local healthcare priorities.

Strong clinical skills, excellent communication and interpersonal abilities, and a commitment to continuous professional development and patient-centred care are essential.

Main duties of the job

The main duties of the role will be providing a patient facing nursing service to the practice population. The role is expected to include but not be limited to:

Chronic disease management (including diabetes, respiratory, hypertension)

Vaccination and immunisation programme (including seasonal vaccinations)

Job responsibilities

  • Act independently within the practice team
  • Manage own clinical practice and that of othermembers of the nursing team to ensure safe effective care
  • Accept delegated responsibility and inherentaccountability for the quality or own practice
  • Assess, plan and evaluate clinical nursing careto meet the needs of the individual and/or identified group
  • Assess, diagnose and treat specific conditionsin accordance with agreed professional protocols and guidelines
  • Take the clinical lead in defined areas ofnursing practice within the primary health care team.
  • Monitor own and teams work areas and practicesand ensure they are free from hazard and conform to safety legislation.
  • Receive direct referral from health careprofessionals, other agencies and patients
  • Plan, manager and deliver programmes to improvehealth and disease in individuals or groups.

Person Specification
Experience
  • Experience of working in a general practice setting.
