The Practice Nurse is a key member of the primary care team, delivering high-quality, evidence-based nursing care in a general practice setting. Working with GPs, nurse practitioners, nursing associates, healthcare assistants, and other team members, the Practice Nurse plays a vital role in managing long-term health conditions, promoting health, supporting screening programmes, and treating minor illnesses and injuries.
The role involves autonomous clinical practice within NMC scope, including chronic disease management (e.g. diabetes, asthma, COPD), wound care, immunisations, cervical cytology, family planning, and lifestyle advice. The nurse also contributes to audits, quality improvement initiatives, and service development aligned with national and local healthcare priorities.
Strong clinical skills, excellent communication and interpersonal abilities, and a commitment to continuous professional development and patient-centred care are essential.
The main duties of the role will be providing a patient facing nursing service to the practice population. The role is expected to include but not be limited to:
Chronic disease management (including diabetes, respiratory, hypertension)
Vaccination and immunisation programme (including seasonal vaccinations)
