Job Description

Job responsibilities: To provide high quality care based on the needs of the practice population which includes treatment, screening, preventative care and patient education. To assess, plan, deliver and evaluate care holistically. To deliver care in accordance with agreed practice protocols and guidelines. Assist in developing new services and complementing and extending the range of services offered to patients.

Key responsibilities include:

  • Provide treatment and care in conjunction with GPs or independently as appropriate and according to policy and guidelines
  • Take responsibility for own clinics and time management
  • Review and filing of own test results and those relating to chronic disease
  • Advise patients on general health and minor ailments with referral to the GP as necessary
  • Immunisations and vaccinations including childhood immunisations; holiday vaccinations and travel advice, inline with GP Contract
  • Venepuncture; First aid for presenting emergencies
  • Diagnostic tests such as ECG, urinalysis, swabs, blood tests
  • Management of patients with chronic disease/long term conditions (e.g., diabetes, CHD, CVA, asthma, COPD, CKD, mental health, learning disabilities, autism, hypertension)
  • Initiate, develop and implement protocols for chronic disease; carry out cervical cytology
  • New patient health checks, well man and well woman checks; health education and weight management advice
  • Family planning and contraception advice; act as chaperone where required
  • Promote health campaigns as appropriate; monitor stocks and immunisation stocks
  • Carry out COSHH risk assessments; keep practice policies and procedures up to date
  • Infection control procedures; cleaning of equipment and waste disposal
  • Use of computer systems to input patient details according to guidelines
  • Contribute to QOF targets; support PCN initiatives; maintain disease registers
Relationships

Responsible to the Partners for clinical issues and the Practice Manager for administrative issues. Work closely with the team, liaising with admin/reception, District Nurses, Health Visitors, Community Matrons, School Nurses, secondary care and social services. Attend monthly multi-disciplinary meetings.

Accountability

Maintain annual registration with the Nursing and Midwifery Council; work within NMC guidelines. Be accountable for own practice, seek training when needed, maintain accurate records per NMC, and maintain CPD and re-validation. Ensure work-related car insurance coverage.

Practice Development

Attend and contribute to internal and external meetings; participate in Practice Development and planning; attend courses at appraisal. Cooperate with research within the practice.

Premises

Be aware of Health and Safety and hazardous conditions; follow fire procedures; maintain a safe environment for staff, patients and visitors.

Person Specification

Qualifications

Essential

  • RGN qualification
  • Current NMC registration
  • Trained in chronic disease management (ideally CHD, diabetes, asthma, COPD)
  • Up-to-date training in childhood, adult and travel immunisation
  • Excellent interpersonal and time management skills; self-motivated and resilient; knowledge of infection control

Desirable

  • Trained in cervical cytology
  • Up-to-date contraception training including administering Depo-Provera
  • Able to administer injections such as zoladex and degarelix
  • Knowledge of EMIS Web; knowledge of primary care targets
