A healthcare practice in Manchester is looking for a dedicated Practice Nurse to deliver effective nursing services to a diverse patient population. The role includes responsibilities for chronic disease management, infection prevention, and collaboration with a team of healthcare professionals. Working as a senior member of the practice multidisciplinary team,the post holder will ensure nursing services are delivered effectively to theentitled patient population, delivering care within their scope of practice, includingto housebound patients.
The Practice Nurse will be responsible for several clinical areassuch as infectionprevention and control, health promotion, chronicdisease management, NHS Health Checks, as well as actively supporting thepractice management team in the reviewing and delivery of clinical policy andprocedure.
Aninterest in diabetes is desirable. Prescribing qualification is also desirablebut not essential (pay scale will reflect prescribing qualification).
St Gabriels Medical Centre will ensure that the post holderhas access to appropriate clinical supervision and an appropriate namedindividual in the organisation to provide general advice and support on aday-to-day basis. There are bi-weekly support meetings with a lead GP.

St Gabriel's Medical Centre are looking for a Practice Nurse to join our team of GP's, Healthcare Assistant, Clinical Pharmacist, Practice Manager and wonderful admin staff to serve our community. We are a friendly, approachable and hardworking team.
You will receive 25 days (pro rata) annual leave as well as bank holidays. Full time (37 hours) preferred but part time applicants (26-36) will be considered. Competitive pay, equivalent to band 5 for non-prescribing nurse and band 6-7 for a prescribing nurse (depending on experience).
The following are the core responsibilities ofthe Practice Nurse. There may be, on occasion, a requirement to carry out othertasks. This will be dependent upon factors such as workload and staffinglevels:
To act as the InfectionPrevention Control (IPC) lead and to work to the IPC Policy providing theimplementation of audits, training, support and evidence of compliance to thepractice manager
To produce the IPCannual statement in conjunction with the practice manager
To develop, implementand embed an effective care management programme for the frail and houseboundpatients registered at the practice
To assess the needs ofpatients ensuring the provision of healthcare is appropriate, incorporatingevidence-based practice
To develop, implementand embed health promotion and wellbeing programmes
To implement andevaluate individual treatment plans for chronic disease patients that promotehealth and wellbeing
To identify, manage andsupport patients at risk of developing long-term conditions, preventing adverseeffects to patients health
As required, to provideroutine nursing care to patients in accordance with clinical based evidence,NICE and the NSF
As required, to diagnoseand manage acute and chronic conditions
To maintain chronic diseaseregisters and support the practice manager with QOF
To prescribe medicationfor patients in accordance with PGDs and own scope of practice
To review patientmedications to enhance compliance
To provide guidance topatients in the use of prescribed and over-the-counter medications regarding sideeffects and dosages
To request pathologyservices and process pathology results as required
To oversee theprovision of chronic disease clinics, delivering patient care as necessary,referring patients to secondary/specialist care as required
To maintain accurateclinical records in conjunction with extant legislation
To ensure SNOMED codesare used effectively by all members of the nursing team, ensuring compliancewith practice recall system
To ensure theappropriate maintenance and use of chronic disease registers
To develop, implement,embed and participate in NHS Health Checks
To ensure compliancewith the practice chaperone policy
To assist GPs withminor surgery when required
To prioritise healthissues and intervene appropriately
To support the team indealing with clinical emergencies
To maintain and managethe emergency drug kit
To implementvaccination programmes for adults and children, liaising with the relevantteams (eg CHIS) and keep cross-organisational system records up to date
To manage and maintaincervical screening results and recalls
To liaise with externalservices/agencies to ensure the patient is supported appropriately (vulnerablepatients etc.)
To delegate clinicalresponsibilities appropriately (ensuring safe practice and the task is withinthe scope of practice of the individual)
To support the clinicalteam with all safeguarding matters, in accordance with local and nationalpolicies
To deliveropportunistic health promotion where appropriate
