To be responsible for theimplementation of processes for the effective management of patients withlong-term conditions using evidence-based practice.
Working as a senior memberof the practice multidisciplinary team, the post holder will ensure nursingservices are delivered effectively to our diverse patient population.
The Practice Nurse will beresponsible for several clinical areas such asinfection prevention andcontrol,health promotion, chronic disease management, health promotion,as well as actively supporting the practice management team in the reviewingand delivery of clinical policy and procedure.
AtThe Arch Medical Practice,we will ensure that the post holder has access to appropriate clinicalsupervision and a named individual in the organisation to provide generaladvice and support on a day-to-day basis.

The following are the core responsibilities of thePractice Nurse. There may be, on occasion, a requirement to carry out othertasks. This will be dependent upon factors such as workload and staffinglevels:
a.To beand effective Infection Prevention Control (IPC) lead and to work to the IPCrequirements by providing audits, training, support and evidence of complianceto the practice manager
b.Toassess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practice
c.Toimplement and evaluate individual treatment plans for chronic disease patientsthat promote health and wellbeing
e.Toidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients health
f.Asrequired, to provide routine nursing care to patients in accordance withclinical based evidence, NICE and the NSF
g.Toreview patient medications to enhance compliance
h.Toco-ordinate the provision of travel medicine services
i.Torequest pathology services and process pathology results as required
j.Tooversee the provision of chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as required
k.Tomaintain accurate clinical records in conjunction with extant legislation
l.Toensure SNOMED CT codes are used effectively by all members of the nursing team
m.To ensure theappropriate maintenance and use of chronic disease registers
n.Toensure compliance with the practice chaperone policy
o.Toassist GPs with minor surgery when required
p.Tosupport the team in dealing with clinical emergencies
q.Toimplement vaccination programmes for adults and children
r.Tosupport patients in the use of their prescribed medicines or over-the-countermedicines (within own scope of practice)
s.Toliaise with external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)
t.Todelegate clinical responsibilities appropriately (ensuring safe practice andthe task is within the scope of practice of the individual)
u.To supportthe clinical team with all safeguarding matters, in accordance with local andnational policies
v.Todeliver opportunistic health promotion where appropriate
In additionto the primary key responsibilities, the Practice Nurse may be requested to:
a.Develop and review audit protocols and processfor the practice, effectively utilising the audit cycle
b.Develop, review and implement nursingprotocols in conjunction with the nursing lead and partners
c.Support all members of the nursing team,providing guidance when necessary
d.Participate in local initiatives to enhanceservice delivery and patient care
e.Support and participate in shared learningwithin the practice in order to improve patient care
f.Continually review clinical practices,responding to national policies and initiatives where appropriate
g.Participate in the review of significant andnear-miss events applying a structured approach, i.e., root cause analysis(RCA)
h. Support thepractice team with the reviewing and implementation of practice policies andprotocols, ensuring conformance to extant legislation
